Objective
To investigate a Middle East respiratory syndrome coronavirus (MERS-CoV) outbreak event involving multiple healthcare facilities in Riyadh, Saudi Arabia; to characterize transmission; and to explore infection control implications.
DesignOutbreak investigation.
SettingCases presented in 4 healthcare facilities in Riyadh, Saudi Arabia: a tertiary-care hospital, a specialty pulmonary hospital, an outpatient clinic, and an outpatient dialysis unit.
MethodsContact tracing and testing were performed following reports of cases at 2 hospitals. Laboratory results were confirmed by real-time reverse transcription polymerase chain reaction (rRT-PCR) and/or genome sequencing. We assessed exposures and determined seropositivity among available healthcare personnel (HCP) cases and HCP contacts of cases.
ResultsIn total, 48 cases were identified, involving patients, HCP, and family members across 2 hospitals, an outpatient clinic, and a dialysis clinic. At each hospital, transmission was linked to a unique index case. Moreover, 4 cases were associated with superspreading events (any interaction where a case patient transmitted to ≥5 subsequent case patients). All 4 of these patients were severely ill, were initially not recognized as MERS-CoV cases, and subsequently died. Genomic sequences clustered separately, suggesting 2 distinct outbreaks. Overall, 4 (24%) of 17 HCP cases and 3 (3%) of 114 HCP contacts of cases were seropositive.
ConclusionsWe describe 2 distinct healthcare-associated outbreaks, each initiated by a unique index case and characterized by multiple superspreading events. Delays in recognition and in subsequent implementation of control measures contributed to secondary transmission. Prompt contact tracing, repeated testing, HCP furloughing, and implementation of recommended transmission-based precautions for suspected cases ultimately halted transmission.

A total of 14740 schoolchildren in seven provinces of Shoa Administrative Region in Central Ethiopia were surveyed for the prevalence of goitre, xerophthalmia and anaemia. Haemoglobin and packed cell volume were assessed in 966 children in one province while an in-depth study was conducted on 344 children in the same province and two others. Goitre, xerophthalmia (Bitot's spots) and clinical anaemia were observed in 34·2, 0·91 and 18·6% respectively of the children. Most biochemical variables were within the normal range while those of haemoglobin (Hb), mean corpuscular Hb concentration (MCHC) and urinary I excretion were lower, and mean corpuscular volume, mean corpuscular Hb (MCH), and immunoglobulins G and M were higher. Hb was strongly correlated with retinol. ferritin, MCHC, MCH, packed cell volume and erythrocyte count while retinol formed a triad with transthyretin (TTR) and retinol-binding protein (RBP) which were all correlated with one another. Total and free thyroxin and total and free triiodothyronine were positively correlated as were the concentrations of the total and free hormones. Thyrotropin (TSH) was negatively correlated with total and free thyroxin and positively correlated with free triiodothyronine. Thyroxin and triiodothyronine in both free and combined forms were all correlated with thyroxin-binding globulin which in turn was negatively correlated with the triad retinol, RBP and TTR. The triad was also negatively correlated with C-reactive protein. Urinary I excretion was positively associated with total thyroxin and negatively associated with TSH. The anaemia found was not nutritional in origin but due to the effect of infestation with intestinal parasites and malaria.
Defect Anisotropy in Movpe CdTe/GaAs

A cross-sectional TEM study of the defect structure in CdTe buffer layers grown on 2°-off (001) GaAs substrates by MOVPE is presented. The nature and distribution of defects in the buffer layers are shown to be highly anisotropic. Images obtained with the beam direction parallel to [110] show that high densities of stacking faults are present on one of the two equivalent (111} planes, whereas those obtained with the beam direction parallel to [110] show subgrain boundaries perpendicular to the interface. HREM images of the interfacial region along these two directions show a pronounced difference in the character of the misfit dislocations with b = a/2 <110>. Those with line directions parallel to [110] are all edge (90°) dislocations whilst the orthogonal set are an intimate mixture of edge and mixed character (60°) dislocations. This anisotropy is attributed to the combined effects of substrate off-cut and the formation of high aspect ratio island nuclei, giving differences in misfit accommodation.
Neutron Rietveld Analysis of Anion and Cation Disorder in the Fast-Ion Conducting Pyrochlore System Y2(ZrxTi1−x)2O7

All information on the site occupancies and atomic displacements which relate a pyrochlore superstructure to the parent fluorite-type subcell is contained in the normally-weak superstructure diffraction intensities. As Ti has a negative scattering length, the supercell maxima in the present phases are up to three times as intense as the fluorite-like reflections, and neutron diffraction provides an especially sensitive probe of the state of disorder. Y2Ti2O7 is found to have a fullyordered anion array and slight disorder, Y0.984Ti0.015, among the cations. In Y2(Zro.6TiO.4)2O7 the oxygen site normally vacant in pyrochlore is half filled and on the order of 15% exchange between cation sites has occurred.

In order to reduce the growth temperature of (Hg,Cd)Te by MOVPE below 350–400°C, alternative organometallic precursors will be required which either decompose at a lower temperature than existing precursors or which absorb strongly at a suitable wavelength in the ultraviolet. The features required for a programme of assessment of organometallics are discussed. UV absorption spectra for dimethyltelluride, dimethylditelluride, diethyltelluride, di-iso-propyltelluride and diallyltelluride are presented and their usefulness as photolytic MOVPE precursors discussed.
